Mechanism

Clobazam enhances the effect of gamma-aminobutyric acid (GABA) at GABA-A receptors, increasing chloride ion influx → neuronal hyperpolarization → reduced excitability → anticonvulsant and anxiolytic effects.

Pharmacokinetics

  • Absorption: Well absorbed orally; peak plasma levels in 1–4 hrs.
  • Distribution: Highly protein bound (~85%).
  • Metabolism: Hepatic metabolism via CYP3A4 and CYP2C19 to active metabolite (N-desmethylclobazam).
  • Excretion: Renal excretion of metabolites.
  • Half-life: Clobazam ~18 hrs; active metabolite ~36–42 hrs.

Side Effects

  • Drowsiness, sedation
  • Fatigue
  • Ataxia
  • Memory impairment
  • Rare: dependence, withdrawal symptoms

Contraindication

  • Hypersensitivity to benzodiazepines
  • Severe respiratory insufficiency
  • Myasthenia gravis

Interaction

  • Alcohol and CNS depressants ↑ sedation
  • Enzyme inducers (phenytoin, carbamazepine) ↓ clobazam levels
  • Enzyme inhibitors (ketoconazole) ↑ clobazam levels

Warning and Precautions

  • Risk of dependence and withdrawal with long-term use
  • Use cautiously in hepatic impairment
  • Monitor for sedation and cognitive impairment
